 When you arrive for your first visit, please be prepared to complete all insurance and health information forms that will allow us to begin your dental treatment at Ellington & Hulbert Family Dentistry. We will ask you to fill out several forms that will get you acquainted with our office.Your initial exam will last approximately one hour. The dental staff will take the X-rays and do a periodontal exam for bone loss and/or poor gum tissue at this appointment. Your dentist, either Dr. Ellington or Dr. Hulbert, will examine your mouth chart, past dental work and diagnose treatment he recommends for the future. |
